Accounts Calendar Hearing Form
 

Due to the COVID-19 epidemic and effective immediately,  the Allen County Superior Court has required all non-evidentiary hearings to be held telephonically until further notice.

As such, Green, Richard and Trent will accept calls between the hours of 9am to 12pm on our regularly -scheduled accounts calendar hearing day. Calls for a hearing will not be accepted after this time for any reason. As an alternative, and due to the high call volume expected, we have developed the following form as a convenience to avoid busy phone lines and confusion.

Please note that this worksheet may be submitted before your hearing day up to 12:00pm of your scheduled hearing date. If we do not receive this completed form before 12:00pm on your scheduled hearing date, you will be cited as in default of your hearing and we may proceed with filing the garnishment order or contempt proceedings.

Please enter your case or cause number that is on the Summons you received from the Court.
Please submit an amount that is reasonable in contrast to the total debt outstanding. For example, if you owe $3,000 and you are employed with no prior withholdings, a reasonable monthly payment amount would be $175. Keep in mind that wage garnishments can withhold up to 25% of earnings. THIS IS NOT A PROPOSAL IN LIEU OF GARNISHMENTS FILED. If you are unemployed or on disability, the minimum monthly payment we can accept is $25.00.
For those choosing Auto Debit, we will mail you an auto-debit authorization form for you to complete and return. You will be able to state when, how often and how much to debit from your account. You may terminate auto-debit plans at any time by calling us.
